After making your appointment

After you have scheduled your appointment, you will receive a message from the service IntakeQ. This is a secure third party service that we use for collecting forms and other information for your visit. When you receive this message, please complete and electronically sign all of the forms and attach copies of the front/back of your insurance card as well as your photo id at least 48 hours prior to your appointment.
