BEFORE YOUR VISIT

Prior to your visit visit there are several documents that you will need to sign and forms that you will need to complete. Everything can be completed online and MUST be completed 48 hours prior to your visit so that we have enough time to review and get everything loaded into your chart.

1. The documents that you need to sign will be delivered to you via a service that we use called IntakeQ. You will receive an email with a code that you can use to access the documents.

2. The forms that you need to complete (Medical history/Family history etc) are located on your patient portal. When you first signed up with our practice, you should have received an email for your portal account. To get to the portal, start by navigating to our website. From there, click on the Patient Portal link at the top of the page and then use your email and password to log in.

   Once logged in to the Patient Portal, hover your mouse over the Questionnaires tile in the left menu.

   Click on Medical History and complete the following five questionnaires:

   1. MFM Patient Medical History Questionnaire

   2. MFM Symptoms Questionnaire

   3. MFM Self-Care Questionnaire

   4. MFM Toxin Exposure Questionnaire

   5. MFM Candida Screening Questionnaire
      

Next, click on the Surgical and Allergies questionnaire and complete.

When you have completed all of the tasks above, you are ready for your first appointment.
